A coordinator uses the note to prepare the right clinician: memory, heart, mobility, comfort, or a general geriatric review. If the date you choose is full, we reply by email with the nearest alternative. Please do not use this form for a life-threatening emergency.
After you submit a valid request, keep the confirmation on screen for your own notes. In ordinary care hours a coordinator writes to the email you gave, confirms the clinician, and lists anything to bring: medicine boxes, a walking aid, or a question the family has been afraid to ask out loud.
First visits last about fifty minutes. Rehabilitation reviews are shorter and often paired with a therapist in the same week. Memory consultations may include a second conversation with a companion who sees the person on ordinary afternoons, because test rooms rarely show the whole day.
